Hiring Case Managers and Panel Coordinators

We’re hiring MSW Complex Care Case Managers, RN Complex Care Case Managers, and Team Based Care Panel Coordinators to support unhoused individuals in the state of Vermont. In weekly sessions, Case Managers will provide mental health services, including addiction assessments, with the support of Panel Coordinators. MSW Complex Care Case Manager

Location:

Hiring in multiple districts within Vermont

Schedule:

Monday-Friday schedule for a healthy work-life balance.

Seeking those with MSW to fill the Complex Care Case Manager position in Vermont. Deliver outreach and on-site case management services to individuals experiencing homelessness at hotel locations.

Responsibilities:

  • Provide holistic clinical case management to individuals with complex needs.
  • Conduct field-based visits in diverse settings to engage and serve program participants.
  • Collaborate with Field Directors and staff for comprehensive care delivery.
  • Implement complex care model using patient engagement tools and shared care plans.
  • Conduct outreach, screen individuals, and facilitate access to health and social services.
  • Participate in case conferencing and shared care planning sessions.
  • Build effective relationships with stakeholders and integrate with healthcare reform initiatives.
  • Apply case management knowledge and community insights for improved service delivery.
  • Use de-escalation techniques and motivational interviewing skills to support individuals.

Qualifications:

  • Master’s in Social Work (MSW) or equivalent with 1 or more years of experience.

Environmental factors:

  • Reliable personal transportation required for travel. Work in motel facilities, offices, and the community. Some tasks may require working outside regular hours. Effective communication and collaboration with stakeholders are vital. Integration with healthcare reform initiatives and critical assessment skills are necessary.

RN Complex Care Case Manager

Team Based Care Panel Coordinator

Location:

Hiring in multiple districts within Vermont

Schedule:

Monday-Friday schedule for a healthy work-life balance.

Coordinate services and collect data for the Team-Based Care program. Track participant progress, facilitate service connections, and collaborate with a multidisciplinary team. Support Field Directors and staff in implementing the program and tracking outcomes.

Responsibilities:

  • Understand team-based care requirements and function as a program team member.
  • Collect and analyze data, prepare reports, and provide recommendations.
  • Contribute to quality improvement and care redesign efforts.
  • Review patient data and consult with care team for accurate service provision.
  • Manage reports and provide necessary data to healthcare teams.
  • Support Field Directors and providers in managing target populations.
  • Proactively reach out to participants and assist with appointments.
  • Support data management, coordination, and patient outreach.
  • Ensure positive patient experience through effective communication.
  • Synthesize and present data to care plan providers.
  • Assist in process mapping and workflow development.
  • Attend meetings and collaborate with the healthcare team.
  • Adhere to policies and regulations, including HIPAA.
  • Report policy infractions and escalate issues as needed.
  • Perform light physical tasks and other assigned duties.

Basic Knowledge:

  • Proficiency in Microsoft Office and electronic health records.
  • Knowledge of process improvement methodologies.
  • Strong understanding of databases and data communication.
  • Excellent organizational and communication skills.
  • Experience with Motivational Interviewing and complex health needs.
  • Ability to collaborate with providers from different domains.

Minimum Requirements/Experience:

  • Bachelor’s degree in a human services field or Community Health Worker certification.
  • Minimum three years of experience in human services.
  • Experience with electronic health records is desirable.
