Position Summary

We are seeking a compassionate and skilled Physical Therapist (PT) to join our outpatient healthcare team. The Physical Therapist will provide evidence-based evaluation, treatment, and rehabilitation services to patients across the lifespan, helping them improve mobility, restore function, manage pain, and enhance overall quality of life.

The ideal candidate is committed to delivering patient-centered care and enjoys working collaboratively with physicians, specialists, and interdisciplinary teams to support diverse and underserved populations.

Patient Evaluation & Treatment

  • Perform comprehensive physical therapy evaluations and assessments.
  • Develop and implement individualized treatment plans based on clinical findings, functional goals, and patient needs.
  • Deliver therapeutic interventions including therapeutic exercise, manual therapy, neuromuscular re-education, gait training, and functional mobility training.
  • Monitor patient progress and adjust treatment plans to optimize outcomes.
  • Maintain accurate, timely, and compliant clinical documentation.

Care Coordination & Patient Education

  • Collaborate with primary care providers, specialists, and other healthcare professionals to ensure coordinated care.
  • Educate patients and family members on treatment plans, home exercise programs, injury prevention, and self-management techniques.
  • Participate in interdisciplinary meetings and case conferences as needed.

Compliance & Quality Improvement

  • Adhere to all federal, state, and organizational regulations, policies, and documentation standards.
  • Support quality improvement initiatives, audits, and accreditation activities.
  • Maintain required professional licenses, certifications, and continuing education requirements.

Professional Practice & Program Development

  • Contribute to the growth and development of rehabilitation services.
  • Promote patient-centered, trauma-informed, and culturally responsive care practices.
  • Participate in staff meetings, trainings, and ongoing professional development activities.

Qualifications

  • Master's Degree in Physical Therapy from an accredited program.
  • Current, unrestricted New York State Physical Therapy license.
  • Current CPR/BLS certification.
  • Strong clinical assessment, communication, and interpersonal skills.
