We are seeking a Registered Nurse (RN) to join an OPWDD-certified residential program in Brooklyn, NY. This role focuses on providing comprehensive nursing care, coordinating medical services, and supporting individuals to achieve the highest possible quality of life.
Conduct comprehensive nursing assessments and evaluations
Develop, implement, and monitor individualized care plans
Ensure accurate, timely clinical documentation
Coordinate with interdisciplinary teams and attend treatment meetings
Monitor medical needs, appointments, and follow-ups
Supervise AMAP staff and ensure compliance with medication protocols
Maintain compliance with OPWDD regulations and standards
Provide on-site clinical support and guidance to staff
Participate in case conferences, trainings, and committee meetings
Serve as liaison between residence and medical providers
Active New York State RN license
Experience working in an OPWDD or similar setting required
Strong clinical assessment and documentation skills
Excellent communication and organizational abilities
Ability to work independently and handle multiple priorities
24/7 on-call rotation required
