Description

We are seeking a compassionate and skilled Registered Nurse (RN) to join an outpatient family care center setting in Rhinebeck, NY. In this role, you will deliver high-quality primary, preventative, and specialized clinical care to patients and families across all stages of life, ensuring a coordinated and supportive healthcare experience.

Key Responsibilities

  • Patient Assessment & Care Planning: Conduct clinical assessments, document vital signs, evaluate patient medical histories, and establish individualized nursing care plans in collaboration with the healthcare team.
  • Treatment & Medication Administration: Administer prescribed medications, immunizations, point-of-care testing, and clinical treatments, maintaining precise medical records and monitoring for side effects or adverse reactions.
  • Care Coordination & Collaboration: Partner with primary care providers, specialists, behavioral health professionals, and community resources to ensure seamless, multidisciplinary care for patients and families.
  • Emergency Response: Provide prompt first aid and emergency clinical interventions as needed, coordinating effectively with local emergency medical services when required.
  • Patient & Family Education: Educate patients and their families on health maintenance, preventative measures, chronic disease management, and treatment adherence.
  • Regulatory Compliance & Records: Maintain accurate, timely, and comprehensive medical records in strict accordance with New York state healthcare regulations and clinic standards.

Qualifications & Requirements

  • Licensure: Active Registered Nurse (RN) license and current registration in the State of New York.
  • Education: Associate’s Degree (ADN) or Bachelor’s Degree (BSN) in Nursing from an accredited institution.
  • Certifications: Current CPR and First Aid certifications required.
  • Experience: Experience in outpatient clinics, family medicine, community health, or ambulatory care settings preferred.
  • Skills: Strong clinical assessment capabilities, excellent interpersonal and communication skills, ability to work effectively within a team, and strong organizational management.
