We are actively seeking talented and dedicated Licensed Practical Nurses to join our growing team within our premier nursing and healthcare facilities in Staten Island, New York. This is an opportunity to provide high-quality direct patient care and positively impact the quality of life for our residents.

Key Responsibilities

The Licensed Practical Nurse (LPN) will work under the direction of a Registered Nurse (RN) and/or Unit Charge Coordinator (UCC) to perform essential nursing duties:

Direct Resident Care & Administration

  • Provide direct, comprehensive nursing care, including the accurate and timely administration of medications and specified treatments.
  • Perform and assist with various diagnostic procedures as ordered by the physician.
  • Assist the designated Registered Nurse (RN) with the planning, implementation, and evaluation of resident care within the nursing process framework.
  • Cooperate effectively with physicians, interdisciplinary teams, and other departments to ensure holistic resident care.

Documentation & Observation

  • Observe, record, and promptly report changes in resident conditions (e.g., vital signs, responses to medications, wound status, incidents/accidents, resident behavior) to the supervising RN.
  • Accurately and timely document observations and care provided in the Electronic Medical Record (EMR) system (e.g., MDS data, Monthly ADLs, resident behavior).
  • Provide basic health information and education to residents and staff as needed.

Safety, Compliance, & Quality

  • Adhere strictly to facility policies and regulatory guidelines, specifically concerning proper drug handling, controlled substances, and infection control.
  • Maintain an environment that is conducive to the health, comfort, and safety of both residents and staff.
  • Uphold quality of care, quality of life, and resident rights standards at all times.
  • Manage concerns from residents, family members, and friends in a prompt, courteous, and responsible manner.

Unit Support & Professional Development

  • Assist in the orientation of new nursing staff, including Certified Nursing Assistants (CNAs).
  • Assist in maintaining appropriate supplies and functional equipment inventory on the unit.
  • Engage in continuous self-growth and development, attend mandatory in-service classes, and stay current on nursing and resident care trends.

Qualifications & Requirements

  • Active and Valid LPN (Licensed Practical Nurse) license in the State of New York is required.
  • Minimum of 2-3 years of nursing experience in a long-term care or sub-acute facility setting is highly preferred.
  • Strong knowledge of medical terminology, clinical procedures, and healthcare regulations.
  • Proficiency in using Electronic Medical Record (EMR) systems.
