Job Title: Care Coordinator / Case Manager
Location: Monterey and Contra Costa Counties, CA
Pay: $23.50 – $28.00/hour
Openings: 2 positions per location (4 total)

This is a hybrid role based in either Monterey or Contra Costa County, and requires the ability to travel locally for in-person client support.

TemPositions is excited to partner with a mission-driven organization dedicated to improving the lives of individuals and families through trauma-informed, integrated care. They provide vital community services that connect people with mental health, housing, and healthcare resources.

We are seeking compassionate and detail-oriented Care Coordinators / Case Managers to support clients through both virtual and in-person services. This role is essential in coordinating care across providers and ensuring each client receives comprehensive and individualized support. This is an ongoing assignment with the potential to become temp-to-hire.

Key Responsibilities:

Coordinate care and services across providers to support client recovery.

Conduct assessments and develop person-centered treatment plans.

Assist with virtual doctor visits and coordinate transportation for clients.

Document all services using electronic health records (Theranest and MS Teams).

Manage ECM and Community Support caseloads (up to 6 cases/day).

Facilitate connections to housing, social services, Medi-Cal, CalFresh, and more.

Collaborate with multidisciplinary teams and attend team meetings.

Advocate for clients and support crisis intervention as needed.

Qualifications:

Bachelor’s degree in social work, psychology, or related field (Master’s preferred).

Experience in behavioral health case management.

Knowledge of trauma-informed and recovery-oriented services.

Strong organizational and communication skills.

Familiarity with Medi-Cal, CCAH, and virtual communication tools is a plus.
