Description
Seeking a Program Coordinator to support day services (Day Hab) for individuals with intellectual and developmental disabilities (I/DD). The role focuses on Life Plan/person-centered planning, documentation, OPWDD and Medicaid compliance, quality assurance, admissions, and interdisciplinary coordination.
Responsibilities:
- Coordinate Life Plans, Staff Action Plans, and person-centered services
- Review and maintain program documentation for accuracy, completeness, and compliance
- Monitor PrecisionCare/EHR records and ensure documentation supports Medicaid billing
- Conduct quality assurance audits and track corrective actions
- Ensure compliance with OPWDD, Medicaid, HIPAA, and agency requirements
- Coordinate admissions, transfers, and discharges
- Collaborate with Care Managers, nursing, clinical staff, families, and interdisciplinary teams
- Participate in Life Plan, clinical, medical, and team meetings
- Support audits, surveys, and corrective action plans
- Identify documentation/workflow issues and recommend process improvements
- Promote person-centered practices and ensure services align with individuals’ goals, preferences, and outcomes
Requirements:
- Bachelor’s degree in Human Services, Social Work, Psychology, Healthcare Administration, or related field
- 2+ years of experience working with individuals with I/DD
- Experience with OPWDD regulations and documentation
- Knowledge of person-centered planning and Medicaid requirements
- Strong organizational, communication, and documentation skills
- PrecisionCare/EHR and Life Plan experience preferred
Hours: Monday through Friday 8:00AM to 3:00PM.
Location: Staten Island, NY 10314.
