Specifically, this role supports an Adult Day Health Center (ADHC) that serves as a vital community haven for Russian-speaking older adults and individuals with disabilities, offering person-centered care for over 25 years.
Location: San Francisco, CA 94115
Pay Rate: $59.29 per hour
Schedule: Part-Time | 16–24 hours per month (Approx. 4–6 hours per week)

We are seeking a dedicated Physical Therapist to manage services for participants of the L’chaim ADHC. This is an ideal role for a clinician seeking meaningful, flexible work in a community health setting. You will ensure participants receive individualized treatment plans and restorative therapy to achieve maximum independence and well-being.
Conduct initial and six-month physical therapy assessments and update care plans.
Design restorative and maintenance programs in compliance with Title 22 and CBAS regulations.
Provide therapy interventions including gait training, pain relief, and mobility support.
Document assessments and progress notes accurately within Electronic Health Records.
Collaborate with an interdisciplinary team (nursing, social work, etc.) to promote holistic health.
Train staff on maintenance exercise programs and safety practices.
Active California Physical Therapy License.
Bachelor’s Degree (or higher) in Physical Therapy.
Minimum of 2 years of experience working with frail or older adults.
Current BLS and First Aid Certification.
Strong clinical judgment and proficiency with Microsoft Office/EHR.
