Job Overview
Seeking a Licensed Practical Nurse (LPN) to support children and families in a community-based Preventive Services Program. This role focuses on health care coordination, family education, home visits, and connecting families with medical and community resources.
Responsibilities
- Coordinate healthcare services for children and families
- Conduct home visits and provide health education and support
- Review medical reports and coordinate follow-up care
- Assist families in accessing healthcare and community resources
- Collaborate with social workers, physicians, schools, and community providers
- Monitor children's healthcare needs and maintain documentation
- Accompany families to appointments as needed
- Support health monitoring and care planning for high-risk families
Requirements
- Current New York State LPN License
- Experience in community-based nursing, home care, care coordination, behavioral health, or pediatrics preferred
- Strong communication and organizational skills
- Ability to travel locally for home visits and appointments
