Description

This role is a Case Planner in a foster care/child welfare program, responsible for managing cases for children, foster families, and biological families with the goal of achieving permanent placement as quickly as possible.

Core responsibilities:

  • Assess each child and family’s needs and create/implement treatment and service plans. 
  • Provide hands-on case management: advocacy, problem-solving, referrals, and life-skills support.  
  • Maintain regular contact with children, foster parents, and biological families, including facilitating visits and supporting relationships. 
  • Coordinate with schools, service providers, and community resources to track progress and address barriers. 
  • Prepare for and attend court hearings, including testifying when needed. 
  • Plan for discharge/permanency by connecting families with community services. 
  • Complete extensive documentation (reports, plans, progress notes) and ensure compliance with agency and ACS requirements. 
  • Monitor children’s well-being (education, health, recreation, independent living skills). 
  • Participate in meetings, trainings, and supervision. 

Work environment & expectations:

  • Field-based role with frequent travel, home visits, and flexible hours (including evenings/weekends). 
  • Requires collaboration, strong organization, and consistent documentation. 

Qualifications:

  • Bachelor’s degree (BA/BSW) with at least 2 years of human services experience. 
  • Child welfare or mental health experience preferred; Spanish bilingual a plus.
