Schedule- Monday-Friday, 8AM-4PM, with occasional weekend availability(Saturdays) as needed, for on-call rotation, with the other RNs on the team.
Clinical Setting--OUTPATIENT Clinic-Non-profit special needs organization, affiliated with the NYC, Department of Education
Servicing adults with various forms of developmental disabilities within an outpatient setting, ranging from mild-to-severe. The RN will have assistance from Certified Nursing Assistant, Interns, and other staff (Special Educators, Clinical Directors, etc.) daily.

Overview
Working under direct supervision of the clinical director, Registered Nurses will be responsible for evaluating, assessing, developing and implementing individualized treatment plans, based on the specific needs among their patients within their assigned caseload. The RN consistently evaluates the patients, once the plan is implemented in to their daily routine, assessing and documenting the patient's progress, and making the proper revisions to the treatment plan as needed.
Documents any progress, or signs of regression, via EMR (Electronic Medical Records), using the software, EPIC. This person will have Certified Nursing Assistants that they will utilize for additional tasks as needed, to assist with documentation, toiletry duties (as needed), etc.

They are consistently evaluating and assessing new and innovative best practices and procedures to assist with the transitioning of patients into their local communities and assisting them in becoming productive members of society. The treatment plans will include the following; working with the patients on implementing basic daily living skills, independent living skills, paying bills/budgeting, into their local communities; seeking out job opportunities, preparing them for interviews, enrolling in community, volunteer events, extra curricular activities, etc.
OVERVIEW OF JOB RESPONSIBILITIES
The duties and responsibilities of the Registered Nurse – Day Programs and Clinic will include, but are not limited to the following:
Serves as a member of interdisciplinary team and is responsible for coordinating and following through on provisions of health care services for assigned program(s).
Arranges medical services for persons served and issues medical referrals as needed.
Provides follow-up on medical problems for persons served.
Conducts training for staff in medical administration, personal hygiene, basic first aid, dental hygiene, infection control, adaptive equipment, vital signs, signs and symptoms of illness and other in-services as may be necessary to meet needs of persons served.
Handles control of communicable diseases and infections through identification and assessment, reporting to consulting physician and implementation of appropriate protective and preventive measures and staff training in regard to infection control.
Participates in interdisciplinary team meetings and annual reviews as required by each division.
Monitors required staff health records and provides tests in accordance with regulatory agency requirements.
Provides immediate first aid with referral to outside emergency services or private physician.
Participates in routine individual case reviews.
Prepares and maintains medical documentation on each person served
Administers medications when appropriate, and supervises certified staff in administration. Supervises storage of medications.
Provides supervision and training for Licensed Practical Nurses and/or less experienced Registered Nurses during their tenure at agency.
Provides first aid and attends to emergency needs of persons served. Maintains Nursing office and orders supplies as required. Transports persons served to appointments as necessary. Attends team meetings and required in-service as scheduled. Visits persons served at home as requested by Director or designee. Keeps abreast of changes in Nursing field. Performs other related duties as requested.
QUALIFICATIONS/REQUIREMENTS:
Valid New York State Driver’s License is preferred, NOT REQUIRED
License and current registration as a Registered Nurse in the State of New York
CPR, BLS, ACLS course certificate
*If expired these certifications can be easily renewed online, (must obtain within the first 1 year of employment.
Ability to read, write, speak and understand English
Good oral and written communication skills
Ability to multi-task, strong organizational and confidentiality.
Interpersonal skills. required for communicating goals/objectives set within individualized treatment plans and daily progress.
Must maintain strictest confidentiality of all assigned patients within caseload.
Must have working knowledge or exposure to working with individuals who are required to utilize adaptive equipment such as; wheelchairs and adaptive devices.
Must be willing to work at various program sites (all local to one another, within walking or subway distance, reimbursed for any form of public transportation needed)
EDUCATION/EXPERIENCE
NYS, Registered Nursing License/ Graduate of accredited nursing program.
(BS in Nursing is required)
One year of experience working with the special needs population is PREFERRED, NOT REQUIRED
