Job Duties and Responsibilities:
Supervision:
- Provide weekly group/individual supervision to Care Managers to ensure that the six health home core services are implemented to meet the needs of all individuals that the program serves
- Participate in meetings as needed to support Care Managers, which can include travel within the service area (Bronx, Manhattan, Queens & Brooklyn).
- Organize and participate in team meetings, documentation meetings, and other programmatic meetings as needed
- Provide initial training to new Care Managers
- Identify training needs and collaborate with Program Director to develop training sessions
- Facilitate activity- and discussion-based groups with Care Managers
Care management services:
- Maintain a small caseload (max 5 clients) to provide care management services, which involves making field visits to perform monthly required contacts based on level of care, completing assessments, Plan of Care, and CANS-NY every 6 months
- Complete documentation in a timely manner
- Submit relevant documents for monthly billing
- Complete required training annually and maintain CANS-NY certification on a yearly basis
Quality Assurance:
- Collaborate with the QA team to ensure team compliance with all documentation, trainings, and agency expectations
- Review charts and documentation (CANS-NY, assessments, notes, Plan of Care) to provide feedback and training for Care Managers
- Assist with internal audits, such as peer chart reviews and intake checklist reviews
- Review incidents and document follow-ups as needed
Other duties:
- Assist with tracking referrals, outreach, and case assignments
- Cover caseloads as needed
- Other duties as assigned
Reporting Relationship
Reports to: Program Director, Child and Adolescent Care Coordination
Qualifications:
- 3+ years of supervisory experience
- Master’s degree in social work, counseling, psychology or related area of study
- Licensure in NYS as LCSW, LMHC, LMFT or LMSW
- Familiarity with best practices in care management and health home programs
- Culturally competent and committed to racial equity
- Proficiency in Microsoft Office Products (Word, Excel, PowerPoint, Access)
