Our client, one of the leading agencies in foster care is looking for dedicated Case Planners to work in the 5 boroughs. With the goal of permanency for children either through reunification with parents, or through adoption or preparation for independent living, Case Planners will manage the process for placement of these children. You will be trained and supported in implementation of three evidence-based models used to support families and caregivers.
Conducts birth home and foster home visits in addition to field visits; provide referrals to community service providers in accordance with the service plan for the family; accompanies families to drug treatment, medical, school, housing, welfare setting; works with children in onsite care program; attend court hearings in a timely manner with prepared court reports and/or permanency reports. Any other duties as assigned.
The ideal candidate must have a Bachelor’s degree in Social Work, Psychology, or Sociology
Experience within Child Welfare and bilingual English/Spanish preferred
