Case Planners provide the essential planning of care to children and families needing a safer environment.
The Case Planner in the Foster Care and Preventive Services program maintains strong social contracts that explicitly emphasize family permanency planning and the responsibility of all stakeholders to accept nothing less than safe, loving and permanent homes for all children and ensures that every child is living in a nurturing, safe, and permanent family as quickly as possible.
- Creating partnerships with families, service plans, educational and emotional support.
- Monitoring the safety, well-being and progress against other critical outcomes by facilitating regular casework contacts and documenting in progress notes.
- Participates actively in Family Court Hearings, Family Team Conferences and other meetings to discuss permanency for children and families.
- Bachelor’s Degree
- Excellent verbal and written communication skills
