Our client is seeking a Case Manager, for a temporary project with experience providing professional intervention, and critical services to clients. Case Manager duties consist of assessment, crisis, intervention and financial assistance.
Responsibilities:
- Conduct comprehensive assessments to identify the social, emotional, and financial needs of clients and their families.
- Advocate for clients by connecting them with appropriate community resources and support services.
- Perform regular home and field visits with clients, adhering to contract requirements and working across multiple assigned sites.
- Maintain accurate and up-to-date client records in both physical files and electronic databases.
- Provide ongoing case management services, including eligibility assessments and service coordination.
- Develop, implement, and monitor individualized service plans to deliver a full range of social service interventions.
- Collaborate with team members and partner agencies to ensure clients receive timely and effective assistance.
Qualifications:
- Bachelor’s degree in Human Services, Sociology, Psychology, or a related field (or equivalent experience).
- Minimum of one year of experience in a social services or case management setting.
- Strong organizational, communication, and computer skills.
- Must be able to conduct home and field visits and travel throughout the five boroughs.
- Full-time schedule: Monday – Friday.
