Limited Time Bonuses for the Following Positions:

RN |  PT | OT | ST |  Health Aide

To be eligible to receive the bonus you must:

1. Complete the pre-registration form below.
2. Complete credentialing by June 1, 2017.
3. Be among the first 75% to complete fingerprinting in August of 2017.
4. And after 80 hours of work (by December 1, 2017) you’ll get a $500 bonus if you indicate Full Time when you pre-registered.
